Insurance Cards & Copays
If you do not have your insurance card and copay at the time of your visit, your appointment will need to be rescheduled.
New patients will not be seen without an insurance card.
Your copay is a contract between you and your insurance company. They require a copy for each office visit billed to them.

Change of Insurance
Please let us know if your insurance has changed.
If you fail to do so and receive a bill, you are responsible for the charge. - 
      
      
      
        
        
New Patients
All new patients with Essence and any other HMOs must have changed their doctor’s name to Dr. Pernikoff, otherwise we will not be able to treat you.
Dr. Pernikoff’s name must be on your insurance card.

Balances on Accounts
All overdue balances must be paid at the time of visit.
If balances are not paid, it will move to our collection department.

Returned Check
A $35.00 fee will be assessed for all returned checks, along with a bank fee. Returned checks must be paid in cash.

Missed Appointments
A $45.00 charge will be assessed for any missed appointments or same-day cancelations.

Late Arrival
If you are late for your appointment, check with the front desk to see if we can accommodate you. Your appointment may need to be rescheduled.

FMLA
The fee for FMLA is $60 per request. The fee will need to be paid prior to the completed paperwork. Please allow 5-10 business days to complete.

Controlled Drugs
When requesting a controlled drug, we will need 72 hours notice to send. If you do not notify us within 72 hours, we cannot guarantee they will be at your pharmacy when you need them.
All controlled drugs require an appointment every three months.

Aggressive Behavior
Our office is a healing space. Aggressive behavior will not be tolerated and can result in prosecution.

Nurse Practitioners
A Nurse Practitioner is a healthcare professional with advanced nursing training who can diagnose and treat conditions, prescribe medication, and manage care.
If you are scheduled to see a NP and prefer to see Dr. Pernikoff, you will need to reschedule.
